Our Advanced Care Team provides:
- Immediate access to medical advice via phone and portal and direct access to providers as needed.
- Arranges referrals to specialists and facilitates exchange of information between our office and the specialist.
- Follows up with patients to make sure they are satisfied with their referral.
- Helps patients find community programs.
