Please bring
- all the medications that you are taking or bring a list of the medications that describe the dosage, frequency taken, and the prescribing provider.
- your insurance card
- if you have been referred by another doctor and you are to bring records, please have those records with you. Without these records, your appointment may be significantly delayed, and may even need to be rescheduled.
